Complimentary Discovery Call
The purpose of the discovery call is to learn more about your unique health concerns and assess your readiness for change. Following our discovery call, I will review your health concerns and determine if we’re good match for one another. Then, you will be invited to schedule your First Visit.
Preparing for Your Initial Visit
As a new patient, you will need to register on the “Patient Portal” and complete the “New Patient Questionnaire.” This information must be completed 1 week prior to your visit. During your initial visit, we will review your health history, goals, discuss next steps, and schedule necessary diagnostic tests or labs. This visit has been divided into two sessions- the first portion via Telemedicine and the second in the comfort of your own home. Combined, this visit typically lasts 120 min.
INITIAL COMPREHENSIVE VISIT: $600
CHILDCARE TIP
If you have children that are older than 6 months, I recommend you arrange for childcare so we can make the most out of our time together. Consider this self-care, sacred time … for YOU.
Follow-up Visits
Once your health history, life story, and lab-work have been gathered, we will meet for a follow-up visit to review the results. Based on these findings, I will create an integrative healthcare plan. Together, we will decide the next best steps for you. This visit lasts 30 min.
FOLLOW UP: $150
Email Visits
Email visits are messages that require a significant change in care but don’t necessitate face-to-face contact. For example; a new prescription or adjustment in dosage. This visit is 15 min.
EMAIL VISIT: $75
Health Coaching
After the health investigation is complete, we transition into coaching. Lifestyle changes can be the most challenging and complex to achieve. A balanced body, mind, and spirit are the essential foundations of a healthy lifestyle. Together, we will find a way to meet your goals and improve your health so you Flourish. This visit lasts 45 min and is virtual via Zoom.
HEALTH COACHING SESSION - $225
Getting in touch
Direct emailing or messaging through the patient portal are the best ways to reach me. For more urgent needs, you may also call the office line. You can expect to hear back from me within 72 hours. If you have an emergent or life threatening concern – you should call 911 and notify your primary care provider.
Note: If you have recent diagnostic tests or lab-work that relates to our work together, you may fax or scan/email to Flourish.
Cancellations:
If you are sick or need to cancel/reschedule, please notify me promptly via email and/or telephone. Since my time is limited, I ask that you provide 7-days’ notice to cancel for new visits and 24-hours for follow-up or coaching appointments. This means that if you cancel less than the allowed window, you will be held responsible for the visit fee. I realize there can be extenuating circumstances and will take these into consideration.
Invest in Health:
Flourish operates as a self-pay clinic and does not accept insurance. My expertise, time, and home-visit model would not be supported by conventional insurance carriers. I will provide you with a superbill (detailed, coded invoice) which you can use to submit a claim to your insurance company or health savings account (HSA) for reimbursement.
Lab-work:
I use LabCorp for routine/conventional labs and most insurance carriers will provide some coverage for these tests. As an integrative medicine provider, I also use many specialty lab companies such as EpigeneticsRx, Genova, Great Plains, and Precision Analytical. Insurance companies often have poor coverage for specialty labs but you can request reimbursement from an HSA account. Cost of lab-work ranges from $50-$2,000.
